Leadership Survey
Use the scale given below to indicate how much each statement applies to you in swimming.
0 - Never
1 - Only a little
2 - About half the time
3 - Most of the time
4 - Always
Use a separate sheet of paper to write the statement numbers and your score for each, adding up your total at the end. After taking the survey for the first time, read the statements once per day for a period of time including at least two meets, making it your goal to apply each statement to swimming during that time. Then take the survey again to see how much you have improved. Suggestion: Challenge two or more teammates to do the same, and talk about the experience with them as you go through it.
